Lots: Bleeding, infection, disease transmission if everything is not sterile. Once in broken teeth, worn teeth, gum recession. If you are still going to do it, use plastic ends on the post.

Infection.: You could get infection around the piercing, chip teeth from the ring or affect your taste buds.

Lots!: Tongue barbells can chip or fracture front teeth. Gums can be damaged by the rubbing of barbells requiring periodontal surgery. If the barbell becomes unscrewed it could be aspirated into the lungs. Infections can be quite severe & eventually lead to disfigurement. There can be significant blood loss & permanent neuralgia (pain).
