Depends: Appendicitis is highly variable and is very different for each person. For some people the abdominal pain begins all over the abdomen region and then over several hours or a day the pain moves to the right lower abdomen. Others may get pains that move toward the right lower back, and others can have a ruptured appendix with little warning. Everyone is different.

Hours. : Usually one feels a stomach ache type feeling with nausea and maybe vomiting. Like a stomach flu. And then within hours it moves to his/her fight lower abdomen. It becomes severe and any movement, cough, sneeze etc makes it worse. (peritonitis. ) can rupture if not removed in the next 12-24 hours.
