See below: Paraphimosis is when a tight scarred foreskin (phimosis) has been pulled back or retracted over the head of the penis (glans) and is so tight that it can not be reduced or returned to its natural position covering the glans. This causes more pressure and swelling of the glans, making paraphimosis dangerous. Phimosis is inconvenient, paraphimosis is an emergency. Unclear what "tip on head" means.

Unsure: Phimosis means your foreskin is tight and may severe enough that you can't retract (pull back or foreskin). This would mean you can never the glans (or head) of the penis. Hypospadias (abnormal position of the urethra on the penis) can result in an abnormal appearing penis. Your doctor can answer your question with a quick exam.
