Most do: Many surgeons like having a signed release on the chart to give blood when indicated. However, i can't remember a time in decades that my ENT consultants gave a kid blood during or after a t& a operation.

Talk to doctors: Tonsillectomy and adenoidectomy surgeries can cause significant blood loss. I doubt that an ENT surgeon will guarantee no transfusion. However, talk to them to explain your concerns. Some doctors use oral steroid medicine (prednisone) to shrink adenoids and tonsils. Something like that from your pediatrician might even put off the need for surgery. Talk to your pediatrician / family doctor.
