As for referral : Try to see a doctor that someone that you know has already seen. Ask them how there experience was, this way it will reduce your anticipatory anxiety about seeing him.

Self help: You could always start with self help books on managing anxiety. It could improve your anxiety enough to get you into see a therapist.
