Surgically: If the ovary was trapped in an obturator space hernia (very, very rare), the ovary may have been compromised and needs to be removed.

Obturator foramen: This is a rare hernia usually found in elderly females. Your are 45 so you are not elderly. The obturator foramen is a hole in your pelvis. It allows structures like the obturator nerve to pass through this area. Intestines and ovaries can be trapped in this area. The correction requires removing the trapped tissue and patching the area to prevent more things getting trapped in the future.

Repair: Anything can poke through a defect in the abdominal wall or "herniate", like omentum intestine, or even an ovary. The treatment is to remove the herniated structure, whatever it is and repair the defect so it does not recur.
