Family practice : When you cough you can rupture small blood vessels. It can then show up in phlegm. It may be from something else like TB it is less likely unless you are at high risk.

Needs further workup: Specks of blood in phlegm may indicate a more serious condition like cancer, or other infectious conditions like tuberculosis. Further investigation with imaging studies like ct scan (cat scan) would be indicated. Make sure your doctor is aware of this finding and workup is started as soon as possible.
