May not be just...: Cosmetic. An appearance such as this may be a manifestation of a pilonidal cyst, the treatment of which is in the realm of general surgery. Make sure you see a qualified experienced surgeon so that you have the correct diagnosis and procedure. Good luck.

Sacral dimple: In general, if these dimple is not too deep and is easily cleaned, nothing needs to be done. It is not the etiology of a pilonidal cyst. Pediatric surgeons used to remove them years ago but now, do not, unless there is a clincal reason to do so.
