Risk versus benefit: We use zyprexa (olanzapine) in pregnancy, when the risks outweigh the benefits. It is a category c medication, which is what many medications that we use in pregnancy are. There are no known negative fetal effects but the data is limited. Often when patients need psychiatric medications outside of pregnancy, it is important to continue them in pregnancy, as it is a time that otherwise many deteriorate.

Possibly: Please discuss the benefits of using this medication vs risks of illness in pregnancy without it, with your psychiatrist and OB doctor. However, a prospective study with this & other atypical antipsychotics vs controls without such exposures, showed no differences between groups in terms of risk for fetal malformations or rates of obstetrical or neonatal complications. Larger studies needed.
