I suffer from obsessions and am on lexapro (escitalopram) 40mg qd, fluvoxamine 400mg qd, remeron 30mg n. is this alright and what augmentation strategies are there?

Get second opinion: Ocd is hard to treat sometime and multiple meds are needed. But psychiatrists shy away from using two medications from the same class, in your case fluoxemine and lexapro (escitalopram). But I do understand each case is different, get a second opinion. Addition of cognitive behavior therapy may help too.

Probably: Lexapro (escitalopram) and fluvoxamine are both serotonin agents and thus have the potential, when combined, to cause serotonin syndrome. However the doses you are taking are not very high so you are probably fine. There are several augmentation strategies but one worth mentioning is adding an atypical antipsychotic (zyprexa or olanzapine, seroquel or quetiapine, among others).

Too much meds: You are on 3 different antidepressants and the dosages of Lexapro (escitalopram) and Luvox are significantly high. Your current combination puts you at high risk for serotonin syndrome, increased anxiety, agitation, insomnia, severe GI problems, neuroleptic malignant syndrome, myoclonic jerks, p450 enzyme inhibition, etc. Cbt is considered the best treatment for ocd. Small dosages of antipsychotics may help. Thx.
