Depends: Consider checking for mthfr mutation. Also consider reading nutrient power. Flushing Niacin in a low dose of 50 to 100 mg should be safe. But she may need other things.

None, actually.: Niacin could be toxic to the liver, and alcohol is of course toxic to the liver. The cumulative effect on the liver is worrisome. I would not recommend Niacin supplementation to someone who is an alcoholic.
