Muscle spasm: Radiation in that direction does not follow a nerve path and thus it is most likely muscle spasm. I would see your doctor and ask about being referred to physical therapy.

Could be rib: You may also have either neck issues- arthritis at the facet joints OR a dislocating rib- see a spine doc and/or a chiropractor who can easily determine which
