I've been having upper left rib pain for three days now. its tender and i have restrictive lung disease and gerd. had asd repair age 5. should i go er?

Probably not: Chest pain of 3 days in a 21 yr old, associated with tenderness is not an emergency. See you doctor, or go to an urgent care if you don't have a doctor.

Depends: If you recall having some sort of trauma to that area and or there is localized tenderness over the painful area then you can probably wait. If no tenderness and you can't recall any trauma or movement that could've caused the pain then you may need to get it checked out. ASD repair at 5 is most probably unrelated to your current symptoms.
