Ask his MD: Schizophrenia is a very serious and chronic mental illness that is often difficult to treat. Patients often stop their meds. So it is important to watch him take his meds. To address this problem there are long acting inject-able meds. Which can be given every 2-3 weeks. Clozaril (clozapine) is considered one of the most effective drugs for this illness but it can cause serious side effects. Ask his md.

Options: Takes medicine? If yes then Clozapine should be considered as a treatment option of other medications have proven ineffective if no, then consideration should be given to long acting antipsychotic by injection also insure other conditions are treated including substance use disorder.
