Easy answers first: Go to a good women's clothing store where they do underwear fittings (i.e., nordstrom), and see what can be done with a new bra. This might help you look better in clothing, but won't help much without! if this doesn't give you the results you're hoping for, see the surgeon who did your surgery. He/she may be happy to work with you to give you a result you like better. If not, get a second opinion.

A Good Start: A properly fitted bra is a good start. It may not reverve the process, but it should help kepp it from progressing. Breast tend to move apart because they follow the curve of the ribs. A bra can help conterat this. Stomach sleeping is another common cause of diverging breast implants. The constant pressure will push the breast implants to the sides. Follow up with your/a plastic surgeon.

Cleavage: Cleavage is created by a good bra, not by the breasts or the implants. If you had a prominently wide sternum (breastbone) before augmentation, you will always have a wide gap. If your implants fall into your axilla (armpit) when you lye down, then you might consider capsulorrhaphy to bring the implants closer together. http://www.dassmd.com/breast-enhancements/breast-implant-revision-los-angeles.

More Cleavage Wanted: It is difficult to give you advice regarding revisionary surgery without seeing photos or an examination but there are many good bra's out there that can probably help you push the breasts together more. Best wishes.

At a stable result?: As much as hardening of the implants (contracture) is considered a problem, softening and laxity are equally a problem. In some instances this may be due to the use of implants rather than a lift or combination the two to reduce excess skin envelope. At a year you are likely at a stable result. Bras will not change the outcome at this point. Mastopexy or capsulorrhaphy are indicated if unhappy.

Augmented 'outies': Splayed breasts after augmentation can be corrected by surgery which commonly entails revising the implant pocket and if their is skin laxity as well then a mastopexy or skin tightening procedure will also be needed.

reoperation: You may need a revision surgery as the breast pocket may be over dissected and larger than needed.

Perhaps: Breasts tend to project along the anatomic path that they start on. An implant can magnify this projection. In my practice, i will use a velcro strap that is fitted to my post-surgical bras if i can see there will be a tendency towards this in a particular patient. At one year unlikely this would help. Best to followup with your surgeon to see if the pocket or skin envelope can be modified.

To late: At this point and strapping or special bras that could of influenced breast shape or implant position after a aug is done. If you have implant meal position then hopefully a simple surgery will correct it for you.
