Yes: Physical stress to inflammed joints will increase the inflammation, especially with gout. I was one of the authors of a scientific paper which actually confirmed this point.

No: Gout is a consequence of Adenosine (a nucleic acid) metabolism. Increased dietary carbohydrate does increase your uric acid levels, which can precipitate gout crystals. Working out, producing hyperthermia will reduce your risk of gout as gout crystals precipitate out of an aqueous solution at low temperatures. If you are having a gouty flare, yes your inflammation will become exacerbated.
