15 inches, or 40cm: In general a patient needs to read at 40 cm or 15 inches keeping your reading material closer than this distance means patient needs more power on the reading glasses. Reading at a distance closer than 15 inches might cause eye strains, headaches, blurry virion.

Depends: Whatever distance is comfortable is the right distance for you. If no distance is comfortable, then you need glasses for this task.
