DOUBTFUL: I don’t know of any cases of coronary referred pain into the feet. Is it one foot in particular, or both? It is easy to check peripheral circulation with noninvasive vascular studies. With that ruled out you can start to look for a musculoskeletal cause of the pain. Good luck.

Unlikely: Unlikely that any ischemia or heart pain would just present with foot pain. She still needs regular follow-up with her peds cardiologist after alcapa repair to follow cardiac function. But if she complains of foot pain it's better to check with her pediatrician.
