Still need exams: It could be a dental problem- she may have cavities or poor oral hygiene. Unless you are a dentist and have examined her, you can't be sure. It could also be post nasal drip or some other medical issue. You should take her to a dentist and a physician just to be sure.

Ped. & dentist: See a dentist to confirm that no invisible cavities are present. Talk to your pediatrician to confirm that sinuses are ok as well.

Stomach or Sinuses: Bad breath can come from three sources: mouth, stomach or sinuses. It is just a process of elimination to determine the source of the bad breath. If her mouth is clean then assess if she has nasal drainage or possibly stomach reflux. Consulting with your child's dentist and /or pediatrician may be helpful.
