Remove, or not: They can be surgically removed or, assuming they're not infected and painful, left alone.

Painful?: If they are painful, a dermatologist or surgeon should be able to help you. Generally, sebaceous cysts on the scrotum are considered a cosmetic issue and are not uncommon, particularly as we age. There may be 1 or many. If you are concerned, see your doctor for an evaluation.
