No depends: On how big the baby is in relation to your pelvis, sometimes u do not need n episiotomy and you will not tear, I am sure the nurses in l&d will message and stretch your perineum when u are pushing to provide more room, but if the doc thinks u need help it is better to cut a clean episiotomy rather than tear.

No: Episiotomies are rarely performed these days and some women have no tearing! so, stitches are not always required.
