No direct connection: Caffeine does not cause a change in the prostate itself. However, caffeine and other drugs that partially mimic Epinephrine (esp "speed" and cold medicines that contain sudafed/pseudoephedrine) can have an effect on urination by causing the bladder outlet to contract. This is the opposite of the effect of commonly prescribed BPH meds. So caffeine and similar drugs are effectively anti-flomax.

BPH: Caffeine is a known bladder irritant and can cause or add to urinary urgency and frequency. These symptoms may already exist in the adult male with bph. Caffeine would likely make BPH irritative symptoms worse.
