Get active!: Your MRI from 4 months ago identified potential irriation and injury to the tendon. Now, 4 months later, it sounds as if the pain is improving, with less swelling. This is the typical pattern of resolution of soft tissue sprains. You might consider gentle therapy so you can slowly return back to you pre-injury state. You may also consider using an ankle support brace with vigorous activity.

Stay active: If there is no pain, i recommend that you continue with all activities as tolerated to build back your strength and stamina. Some patients benefit from some physical therapy. I would not be concerned with a little swelling at this point.
