Hole in the heart: Some holes in the heart can be heard with a stethoscope though typically an echocardiogram (ultrasound of the heart) is used for definitive diagnosis.

Yes: The physical exam is and should be the first evaluation that indicates the diagnosis. If the hole is between the pumping chambers it can be quite loud. If between the upper chambers it is softer and due to the extra blood crossing the pulmonary valve.
