Tarsal tunnel: He should be evaluated for tarsal tunnel syndrome and low back involvement as well. Numbness is usually indicative of nerve involvement although there may be other causes.

Back issue?: Numbness, sharp pain sensations are often nerve related and can even be related to a back problem where the nerves to the feet and ankles exit the spine. You mentioned that he has numbness when sitting. Recommend he see an orthopedic surgeon who specializes in spine, neurosurgeon or neurologist to help determine the cause of his symptoms.
