Aerobic exercise: I am sorry that you suffer from chronic stress and depression. I wonder if taking a little less Paxil (paroxetine) (perhaps 30 mg and eventually 20 mg) might actually help you feel better. More is not always better. But, most importantly, if there is any way that you could "make" yourself do aerobic exercise -- like 40 mins. Every other day -- I am sure that you would feel at least temporarily better.

Resistant depression: If on this dose for four weeks or so with little or no improvement consider increasing the dose unless there are significant side effects. Sedation, weight gain, worsened short-term memory, dizziness, and agitation/anxiety. Cognitive behavior psychotherapy may be advisable. Other medical causes of depression have been ruled out? - vitamin, thyroid, medication, hormone, sleep, comorbid illnesses.
