Not necessarily: As long as babies need diapers there are times when the moist environment will trigger a rash.The rest of the skin at that age is going through the transition to the outside world with hormone shifts , irritants & other issues.In the womb they had really good moisturizers, after delivery many take six months before their skin seems stable.

Yes: It may be an allergy/eczema to a formula your baby is taking, or a sign of some other diseases. Visit your pediatrician for a check up.
