Obstruction = Pain: Kidney stones that are within the kidney and not causing any obstruction to the flow of urine are generally not symptomatic. If a stone blocks the flow of urine, pressure builds up above the stone causing stretching (hydronephrosis) and deep pain. That said, sometimes a stone can be obstructing a single calyx (see image) and can cause pain, even though the stone is not within the ureter.

Possible: If the stone obstructs the kidney you can have intermittent pain.

Unlikely: A stone in the kidney that is not causing blockage is unlikely to cause pain. It may cause blockage intermittently and not have shown up on the x-rays.
