Myofascial pain: In my practice, myofascial pain is the most common cause of upper back, neck, and shoulder pain. It is more common after injury such as whiplash, and more common in women. It can flare up after over exertion, with stress, and poor sleep. Shoulder and upper back weakness and slumped posture are partly to blame. A good physical therapist or physiatrist could help with rehab of this condition.

It may well be : Muscle spasms but the question is the cause of these spasms. This needs a further evaluation since it has lasts this long. See an orthopedic spine surgeon for an evaluation as it could all be arising from your neck region or upper thoracic region.
