Many things: There are many reasons for urgency symptoms. Sometimes an infection is present with no bacteria seen because a different agent is responsible for the infection. However, other processes that can cause urgency are a small bladder, external pressure on the bladder (ovarian cysts, uterine fibroids), anxiety, certain medications or an intrinsic abnormality in the bladder called "overactive bladder.".
