Spine problem: Sharp pains in lower back could be signs of punched nerve or herniated disk. In women it could be from ovarian cysts. Gas pains are usually felt in the abdomen.

Varies: There are multiple potential causes. Some patients with chronic back problems can have a spasm that can cause sudden, severe pain. It can be nonmusculoskeletal causes as well such as kidney, bowel, or gall bladder symptoms to name a few possibilities. I would discuss with your doctor.
