Surveylance: There are some screening tests that can be done in pregnancy that are non invasive & give suggestive data. A blood test on you is non specific but can suggest a problem, a high resolution ultrasound can look for some suggestive features, an amniocentesis can tell for sure. Your risk at 38 is ~ 1/270 so you are not as high a risk as you suppose.
