Life-threatening: A penetrating ulcer is one that has eaten through the entire wall of the stomach/duodenum. It can be the cause of peritonitis, or the penetration can be into the pancreas and cause pancreatitis. Both conditions are potentially very dangerous and even lethal. Severe abdominal pain, distension, nausea/vomiting, fever and chills, back and shoulder pain, are some symptoms. See your doctor immediately.

Severe pain mainly: It can feel so severe as to be an emergency. And it can be a cause of bleeding from the stomach which leads to vomiting of blood. It is severe enough to require monitoring in the hospital.
