Needs a full eval: By 2yo a kid should have dozens of simple words that are understood by strangers at least half the time. A basic evaluation of the child's speech & hearing along with a general developmental evaluation should be done to identify treatable issues. This is often available through child development centers affiliated with children's hospitals or university centers.
