The old injury...: May have set up a senario that lead to arthritis. See a doctor for physical exam, x-rays and management of your specific degreee of the problem.

Traumatic Arthritis: I suggest that you see an ankle specialist to further define your problem and start some conservative treatments. If none of the conservative treatments help, there are numerous surgical options (e.g., cartilage replacement, arthroscopy, joint replacement, joint fusion, etc).
