If uncomfortable: Your foreskin is probably too tight when retracted to the extent that it cracks. A tight foreskin is called phimosis and in adults is best managed surgically. A phimotic foreskin can certainly tear if retracted with force. Prescription strength steroid cream can be tried if you want to avoid surgery.
