Pigmented Nevus: Yes it may still be benign. The abcd criteria is more of a screening instrument to identify nevi worrisome for dysplasia or malignant melanoma and is not very specific. A biopsy is your surest way to tell.

Rarely.: Such a behavior is suspicios for melanoma, but benign nevi may become depigmented. Excision biospsy will confirm.
