Nothing's 100%: If you were put on anticoagulants for clots in your leg veins, a piece of clot could break off even on therapy and cause a pulmonary embolus. Having a pulmonary embolus while on anticoaugulation therapy is one of the indications for placement of an inferior vena cava filter.
