GERD and: Reflux are the same. Severe symptoms not relieved by medication require a consult and likely an egd (upper endoscopy).

GERD: It definitely sounds like gerd although i'm surprised the Nexium (esomeprazole) doesn't help. You would definitely be a person i would recommend an egd with a bravo ph probe placement. This is a great test to really see if what you are feeling is reflux and how bad it is. There are good surgical options if it is gerd and the medications don't work.

Maybe not GERD...: To expand on dr. Dugan's prior answer, evaluation is key to determining if your symptoms are reflux-related or not. Also, if reflux, is it acid or non-acid material that is involved? Sometimes, "sourbrash" (as you describe) may be due to pulmonary, ent, or dental issues. If unresponsive to therapeutic dosing of nexium, (esomeprazole) then start your evaluation with the gastroenterology specialist to determine rx.
