Possible: Ir is a possibility, but also an incomplete treatment or an infection with a resistant bacteria such as MRSA may cause it also.

Certainly possible: Especially if sugars poorly controlled, has diabetic neuropathy, reduced blood flow to legs/feet, varicose veins.
