Nerve pain: You could have irritation on the top of your foot from inflammation of the deep peroneal nerve, which overrides the instep of the foot. If you have a high instep and tie your shoelaces tight, this can irritate the nerve. Loosen the laces or tie them in a fashion where you leave a space (tie the laces in parallel rather than acrosss) where the instep is located on the foot.

Several thing: It could be anything from a stress fracture of bone to a nerve entrapment to tendonitis. Needs evaluation by a podiatrist.
