Inflammation in H&N: Painful swollen nodes an indication of an inflammatory process in the H&N area. If the nodes are high in the posterior neck region, examine the scalp for an infection. If in anterior triangle mid neck one should do endoscopy to see if lesion in area of larynx or cords . In supraclavicular area, inflammation apex of lung can cause the problem and chest X ray can help. Bx if necessary.

Infection to tumor: The dx of swollen lymph nodes in neck depends on size, location and symptoms of pain. In upper neck 2 nodes can represent spread of throat infection to ear, spread from parotid gland. In mid neck tumor can arise from metastatic thyroid and in supraclavicular fossa , an upward spread from lungs. Biopsy if persistent or enlarging can show lymphoma, infection or tumor metastatic tumor.

They are palpable: Normal sized lymph nodes are quite small and not palpable. If the nodes enlarge to a size of 1 to 2 cm, then they are palpable if you run down your fingers over the skin of your neck. Very large lymph nodes(over 2 inches in size) can be visible to the eye. But that is pretty late to discover them.
