In general : In general the larger the herniation the more severe the symptoms, but this is not always the case. Every once in a whole we see a very large but relatively asymptomatic herniation, and sometimes a much smaller herniation with significant symptoms. Also 'far lateral' discs tend to be very painful even though they may sometimes be smaller relatively.
