Coumadin (warfarin): Normally we do take you off the medication, with your md permission, for 5-7 days prior to extractions. Depending on your problem, your oral surgeon may have some suggestions to remove the teeth without compromising your health. You may just bleed a little longer.

Dont: Currently it is not recommended that you stop your Coumadin (warfarin) prior to oral surgery. You will need to have your physician check your inr which is a measure of your clotting time. If this number is normal should be ok to have your teeth extracted.

Advice: Each case is different- both medically and dentally. Your treating physician’s recommendations take top priority. Your dentist and your physician should discuss the matter and come to a resolution that is acceptable to both for your best interests. In almost all cases, i can extract teeth without having a patient stop their medications and deal with some increased bleeding at times.

Stay on Coumadin (warfarin): It's unlikely that you would need to stop your coumadin (warfarin). Good surgical techniques can control any bleeding, assuming that your inr data is in range. A fibrin pack in the extraction sites can be useful.

Need more info: It isn't possible to answer your question without knowing why you are on Coumadin (warfarin). If you are taking Coumadin to treat a blood clot (dvt), it would be safer to stop Coumadin than if you are taking it because you have a mechanical heart valve or arterial thrombus, where stopping Coumadin could result in stroke and death. In any case, a vascular md can get you through this safely.
