Dysmenorrhea: Sounds like a gyn problem such as endometriosis. Get a gyn checkup and try starting anaprox (naproxen) just before your period begins.

Many options: Many options for painful periods. 1. Take anaprox (naproxen) 24hrs before your menses start and take around the clock through your worst days. 2. Continuous birth control pills to stop or reduce number of periods per year. 3. Lupron injections to suppress endometriosis. 4. Surgical excision of endometriosis +/- presacral neurectomy. 5. Hysterectomy to definitively treat if no future desire for kids.

Different meds: Depo Provera (medroxyprogesterone) would be a consideration for stopping periods. Birth control pills are considered higher-risk in over-40s, especially smokers; however, many experts encourage the use of hormones, given the precautions to watch for blood clots. If you truly have "dysfunctional uterine bleeding, " you may talk to your gyn about a uterine ablation as long as you are finished having babies.
