Heartburn: This can be secondary to reflux, however burning sensation along spine is not typical of reflux. Given difficulty swallowing this can be either case of reflux, candida, and/or narrowing of esophagus. Given your age above 40 and difficulty swallowing (dysphasia) which is one of alarm signs, endoscopy is recommended to rule out complication of reflux and to screen for barrets(per malignant condition.
