Yes.: Therefore you may have sx of prolapse, but not prolapse. Also, minimal prolapses can heal. Next time you see the doctor, i would ask. Also, i would have assumed you discussed this with him/her on your visit. If not, do so!

Maybe: Your prolapse may not have been obvious to your doctor during the placement of your iud. We often ask you to bear down during exam in order to see prolapse of the bladder and rectum.

Usually: Unless your prolapse is severe it may not be picked up during an iud insertion. Sometimes prolapse is only visible with certain maneuvers that you doctor would have you perform during an exam. Prolapse may also be more pronounced if you are examined while standing. If you are concerned then you should see your doctor for an evaluation.

Not necessarily: When you have an iud placed, you are lying down. Often we don't see prolapse unless you are bearing down during the exam (pushing) or if you are standing up. A focused exam for this problem can be easily done to help with your diagnosis.
