Without: Revascularization the level of dead tissue removal is undefined. In extremity gangrene vascular intervention could save the foot, the ankle or knee. Without it an above knee amputation may be the only option.

Gangrene: Is irreversible but to heal above it you need blood supply. In patients with toe gangrene and perfusion pressure of less than 40 mm hg a mid foot amputation, a b-k amputation will fail. With revasculaization even the foot could be salvaged and a mid foot amputation does not require prosthesis.

HBO/vascular repair: Revascularization surgery and hyperbaric oxygen treatment can potentially improve circulation to the distal leg and foot. On occasion improving the oxygen and nutrient supply to the distal extremity will prevent the need for an amputation. Once the distal extremity is gangrenous though, the odds of tissue regeneration are extremely limited.
