Depends on cause: Swelling of both ankles is due to a variety of diseases including heart failure, low blood proteins, liver failure, pregnancy and other systemic diseases. Swelling of just one leg is due to some type of venous or lymphatic blockage in that leg, or possibly a growth in the pelvis or groin on the same side.
