Yes: Yes- a hiatal hernia if large enough- can cause herniation of contents into the thoracic cavity - that can cause shortness of breath. See a thoracic surgeon for evaluation.

Hiatal hernia: Hiatal hernias can affect lung function if large. Since you are symptomatic i would recommend a consultation with a surgeon soon.
